As an LPN, you'll provide vital nursing services in residential care facilities, outpatient psychiatry, addiction, and housing services. You'll play a key role in maintaining a safe and therapeutic environment for clients while fostering strong relationships with clients and clinic staff.

Key Responsibilities :
Administer medications via various methods, including oral, injection, and self-administration, according to service line requirements.
Perform nursing assessments, including physical and mental health assessments, and psychiatric screenings.
Document medication administration and nursing assessments accurately and completely.
Conduct MAR reviews and medication counts, monitor house stock of medications, and manage medication refills for clients in housing programs.
Communicate with clients regarding medication needs and questions, coordinating with providers for refills as needed.
Assist with client rooming, verify client data and medical history, and perform preliminary physical requirements, such as blood draws, urine samples, and vital sign measurements.
Complete all required documentation in a timely manner, including safety plans, nursing-led educational groups, and interventions with clients.
Maintain accurate medical and medication records and assist with lab and phlebotomy duties.
Oversee infectious disease control and client hygiene, addressing health-related issues under nursing leadership supervision.
Participate in required meetings and perform other related duties as assigned.
Education and/or Experience Qualifications :
Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
Graduation from an accredited practical nurse program.
Additional Qualifications :
Proficient in documentation and information management using written and electronic formats.
Strong communication and problem-solving skills, with the ability to adapt to new situations.
Skilled in reading and interpreting medical documents and producing clear, concise reports.
Strong mathematical competency and ability to follow detailed instructions.
