Job Overview

The Junior Prevention, Education and Outreach Coordinator learns to facilitate, coordinate, implement, and evaluate prevention and community based outreach programs at Children's Hospital Colorado (CHCO).
The Junior Coordinator supports programming by prescribed coordination of class details, including coordinating with the relevant community partners.

Qualifications

EDUCATION - High School Diploma.
EXPERIENCE - Two years of experience coordinating and implementing preventing and education initiatives that help address social determinants of health required.
Three years of experience assisting patients and families or other related healthcare/community experience preferred.
Spanish speaker preferred.
Knowledge of state social services agencies and community resources

Responsibilities

POPULATION SPECIFIC CARE No direct patient care.
ESSENTIAL FUNCTIONS An employee in this position may be called upon to do any or all of the following essential functions.

Collects and uses valid and reliable quantitative and qualitative data.
Contributes to assessments of community health status and factors influencing health in a community.
Contributes to development of program goals and objectives; contributes to implementation of Implements CHCO population health strategies; Gathers information that can inform options for policies, programs, and services Assists with the coordination of program specific outreach events, as prescribed, and maintains program branding materials including brochures and websites.
Suggests approaches for disseminating public health data and information.
Describes the concept of diversity as it applies to individuals and populations; addresses the diversity of individuals and populations when implementing policies, programs, an services that affect the health of a community.
Serves as liaison between program, department, CHCO and community partners.
Describes the programs and services provided by CHCO population health to improve the health of a community.
Supports development and implementation of population health programs based on community benefit strategic plan, including activities designed to improve health, safety and family education/empowerment.
Actively trains and develops prevention, education, and outreach skillsets and progressively becomes responsible for more complex duties and
