Job Summary The hospice and palliative care physician works in collaboration with hospice staff to provide professional, comprehensive, compassionate, quality medical care with a focus on symptom palliation and end of life care to hospice patients under the direction of the Hospice Medical Director.
Care may be provided directly, through visits to the patient's place of residence, facility or, indirectly, by advising and directing members of the interdisciplinary team (IDT).
The position will also identify opportunities to improve care delivery, use resources effectively, and comply with legal, regulatory and organization policies and procedures.
Essential Responsibilities Clinical Services and Care Collaborates with the interdisciplinary team to create, implement, and revise care plans which reflect the patient's goals, values, and mental, spiritual, and physical needs.
Certifies in writing that each individual considered for admission to hospice, or each patient considered for continuing hospice care is terminally ill in accordance with applicable law, regulation, hospice policies and procedures, contract provisions or the patient's benefit requirements.
Medical eligibility for hospice services is determined using available history, diagnostic and prognostic indicators, related diagnoses if any, current subjective and objective medical findings from IDT or face to face to visits as required, current medication and treatment orders, and information about the medical management of any of the patient's medical conditions unrelated to the terminal illness.
Engages in independent decision making about a patient's healthcare concerns and provide hospice services on an as needed basis.
This may include managing urgent medical issues by advising and supporting on-call nursing staff, including after-hours and on weekends or making patient visits.
Provides education, counseling, and guidance to patients, families, and caregivers about medical issues, goals of care, and hospice services.
Helps to coordinates case management activities for patients.
Attends IDT meetings, acting as a medical resource for the team, collaborating with the IDT and assuring compliance with medical standards of care for hospice.
Collaborates with the patient's attending physician and the hospice IDT to review/update the patient's individual plan of care and ensure that the medical needs of the patient are being met.
Provides consultation, support, and education to the hospice IDT, attending physicians, and other colleagues regarding medical eligibility, symptom management, and any other issues related to the care of hospice patients.
In absence of the hospice medical director, the hospice physician assumes role of the Hospice Medical Director to ensure that medical services and support are available to hospice patients and the IDT 24/7, either directly or through arrangements.
