Nursing only Michigan hospitals need their licensure verified through LARA not NURSYs but you will still upload here.
Current license and primary source verification within 30 days prior to start of assignment for all licensed professionals. Nursys quick confirm verifications should be completed for all APRN, RN's and LPN/LVNs. Nursys should be used to review ALL license activity in all states, not just the state of placement.
Documentation of vaccination series completion OR documentation of declination to vaccine
1st within 1 year, 2nd within 30 days of start. And/or Negative IGRA within 90 days of offered start date lab printout prior to start. If positive TB record; Requires Proof of Chest X-Ray within last 1 year to be accompanied by TB questionnaire.
The candidate or associate should provide documented proof of two Varicella vaccines, serological evidence of immunity, or verification of a history of varicella or herpes zoster (shingles) by a healthcare provider.

Required for all travelers. If placement is in a state other than home state/issuing state of compact license for RNs, address on Driver's license must match the home state for the compact license.
