The Home Health Registered Nurse is responsible forproviding and documenting skilled one-on-one nursing care in accordance withthe physician-ordered individualized patient care plan. This care is provided mostly in the patientshome or a residential setting but can transfer outside the home duringappointments and other activities the nurse may accompany the patient to.
Essential Duties and Responsibilities:
• Completes an initial assessment of patient and family todetermine home care needs when applicable
• Provides a complete physical assessment and history ofcurrent and previous illness(s)
• Regularly re-evaluates patient nursing needs
• Initiates the plan of care when directed and makes necessaryrevisions as patient status changes
• Uses health assessment data to determine nursing diagnosis
• Establishes goals based on nursing diagnosis andincorporates therapeutic, preventative, and rehabilitative nursing actions
• Includes the patient and the family in the planning andexecution of the treatment plan and goals
• Initiates appropriate preventative and rehabilitativenursing procedures
• Administers medications and treatments as prescribed by thephysician
• Provides healthcare instructions and education to thepatient as appropriate per assessment and plan of care
• Identifies discharge planning needs when applicable
• Adheres to home health agency policy and protocol
• Attends case conference, team meetings, and educationin-services as required
Minimum Requirements:
• Graduate of an accredited school of nursing
• Currently licensed as a Registered Nurse in the state inwhich the RN will practice
• Complies with all relevant professional standards ofpractice
• Participation and completion of Amergis' Competency programwhen applicable
• Current CPR if applicable
• TB questionnaire, PPD or chest x-ray if applicable
• Current Health certificate (per contract or stateregulation)
• Must meet all federal, state and local requirements
• Successful completion of new hire training as applicable tojob site
• Understand patient confidentiality and HIPAArequirements
• Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
• Computer proficiency required
• Must be at least 18 years of age
