Responsibilities:
Transform the lives of our patients/residents by delivering compassionate care, while building meaningful relationships with patients and their families.
Conduct nursing assessments and collaborate with the nursing team and other healthcare professionals to create effective care plans.
Set realistic, measurable short- and long-term patient goals, monitoring and adjusting interventions to minimize rehospitalizations and evaluate outcomes.
Administer medications and perform treatments according to physician orders.
Delegate responsibilities to staff and supervise the execution of those tasks to ensure high standards of patient care.
Effectively communicate patient information within the assigned team and during shift changes.
Qualifications:
Must be a graduate of an accredited nursing program and hold a current license from the State Board of Nursing.
CPR Certification is mandatory.

Posted Salary Range: USD $34.00 - USD $42.00 /Hr.
