Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient's plan of care and goals
Observe movements, conduct assessments, document progress and modify care plan as neededAssist patient with exercises and hands-on therapy to manage pain and increase mobilityEducate patients on their plan of care, established goals, exercises and self-careCoach family on the plan of care, therapy goals and safely assisting with exercisesA few must-haves for Home Health Physical Therapists:
