Job Title: Rehabilitation Therapist
Job Specialty: Occupational Therapist

Experience: New graduates are welcome; SNF experience preferred
License: State License Verification required
Certifications: CPR (Fundamental Facilities), MMR, Hep B or Declination, Tdap or Declination, Flu shot or Declination
Must-Have:
• Covid test 72 hours prior to start date with results issued 48 hours prior to start date
• Ensure prescreen form is completed and attached upon candidate submission
• Attach resume and license verification for consideration
• Provide three references
• Complete County Criminal check (7 years for all counties lived and worked)
• Complete Federal Criminal check (7 years or more)
• Conduct Multi-State Criminal Search (7 years or more)
• Perform Nationwide Sex Offender Search (7 years or more)
• Complete HHS/OIG Search
• Conduct GSA Search
• Perform Social Security Number Trace
• Complete 2-Step PPD, Quantiferon, or provide Proof of Positive PPD and Chest X-Ray
• Conduct SAM Search
• Complete Maryland State Background check
Note: This position does not require holiday coverage and is not bilingual preferred. The position urgency is normal.
