Successful completion of accredited LPN program.
Ideal candidate will have experience working in outpatient setting, with inter-disciplinary team, and with HIV and LGBTQ+ populations.
Licenses/Certifications:

PA LPN license required.
CPR required
The Clinical LPN for out-patient primary care is responsible for the prompt, efficient and accurate performance of all patient care duties.
LPN will work with IDC (HIV) care teams on provision of comprehensive care to support optimal health outcomes.
Responsibilities will include:

Managing provision of long-acting injectable treatment; coordinating prior authorization, ensuring medications are delivered on time, communicating with patients, and providing safe and expedient medication administration
Providing clinical assistance to primary care providers for procedures and clinical services
Expanding lab services, including bloodwork and rapid-access to STI testing and treatment
Medical triage and advice
Adherence counseling and support
Administrative support with DME requests, referrals, medical forms and records, prior authorizations, etc.
Other clinical support as needed
This position requires providing service to the following age population(s) 18+ in a manner that demonstrates an understanding of the functional/developmental age of the individual served.

Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check.
The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
