This Registered Nurse 4 is assigned to Region 5 Office of Public Health (OPH) and serves as a float lead nurse in the Parish Public Heath Units throughout region 5. The Registered Nurse 4 practices within the scope of practice designated by the Louisiana Nurse Practice Act (URS 37:31), adheres to the polis, procedures and standards of the Louisiana Department of Health/Office of Public Health. The RN 4 i responsible for assisting in directing, planning, and implementing nursing activates for all OPH personal health programs as well a promoting a positive image and working harmoniously with other members of the health care team, Reports administratively and professionally for the RN Regional Manager.

30% Utilizes the nursing process of assessment, planning, implementation, and evaluation in the practice of professional nursing. Conducts nursing assessments on patients according to OPH policies and procedures and program protocols [Maternal Child Heath; Sexually Transmitted Disease (STD); Reproductive Health (RH); Children's Special Heath Services (CSHS); immunization; Genetics; Chronic Disease; Women, Infants, and Children Supplement Food Program (WIC); Infectious Diseases; nd Tuberculosis (TB] in public health program Clinic, communities, shelters, and/or any additional assigned sites designated by the Louisiana Department of Health/Office of Public Heath for public health services. Evaluates the outcome of public health interventions. Duties include, but are not limited to services, which may require visiting homes to interview patients and Caregivers, recording the medical history, checking and recording vital signs, measuring, weighing and recording findings. Assists the physician or RN Advance Practice with examinations, Serves as an STD Nurse performing syndromic assessment and treatment of patient for Sexually Transmitted Diseases according to LDH/OPH Standing Orders.

20% Obtains blood specimens and other specimens for analysis. Interprets laboratory test results. Provides quality controls, counseling, intervention, treatment, and follow up (Le, Newborn Screening igG, HV-rapid and 'conventional testing, AST, sputum for TB, Tspots, urine pregnancy, Hepatitis and C, Pap Smears, Gonorrhea, (Chlamydia, Convalescent bloods-West Nile Virus) and refers as needed, Maintain CLIA/OSHA guidelines in performance of nursing duties. Reviews all laboratory forms and specimen labels for accurate and complete information and makes sure request form numbers match sample numbers. Enter data for bling purposes into the Electronic Health Record with each direct patient service. Audits patient records in the Electronic Health Records per OPH guidelines.

20% Administer/distributes medication (Le, Spectinomycin, Azithromycin, Rocephin, Bicilin, age appropriate Immunizations, birth control methods and vaccines for Hepatitis A and 8). Provides education on medications, side effects, adverse reactions, care and/or referral. Monitors and makes hospital and home visits as indicated for TB patients. Administers/distributes medications for T8 patients and contacts and monitors effectiveness, Provides Directly Observed Therapy (DOT) to TB patients and contacts as needed. Enters data into LINKS (immunizations) computer system. Serves as a collaborative resource in child abuse cases

15% Assists the RN Regional Manager in the development of the Continuous Quality improvement Plan {record reviews, public health competencies, and nursing care performance) for the parish health units.

10% In collaboration with parish health unit and regional leadership, participate in developing strategies to improve the health of residents of Region V. Assist in planning for and participate in community outreach and education events and health unit initiatives aimed at improving health outcomes and promotion of parish health unit services.

5% Provides emergency care within the scope of nursing practice and OPH guidelines such as reviving a person from a faint, cardiac arrest, or anaphylactic shock, removing an obstruction from a person's breathing passage, or administering medication to alleviate an allergic reaction. Maintain CPR certification as required by agency guidelines. Responds to natural and manmade emergencies and public health/bioterrorist threats on a 24-hour bases (hurricanes, flooding, tuberculosis, sexually transmitted diseases, food-borne illnesses, and others as they emerge). During a disaster situation, must report, as assigned to a Medical Special Needs Shelter (MSNS) or to the health unit. When assigned to the MSNS, incumbent is to report to the designated shelter and is expected to prepare personal needs for at least five days. Performs duties assigned by the appropriate Incident Command (ICS) Staff Section Chief.

Accepts and performs additional duties not listed, but necessary for the smooth operation of the work unit or to help insure quality of agency services.
