We are seeking a skilled RN Case Manager to join our team at Elite Hospice in providing high-quality end-of-life care to our patients and their families.
As a Hospice RN, you will be responsible for developing trusting relationships with patients and their families, and for providing expert care and guidance to help them navigate the complex healthcare system.
Key responsibilities will include:
Making initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
Identifying patients' physical, psychosocial, and environmental needs, and reassessing as needed.
Initiating and coordinating plans of care, maintaining accurate and relevant clinical notes, and documenting problems, goals, interventions, and patient response.
Collaborating with patients, families, attending physicians, and other healthcare providers to provide comprehensive care.
To be successful in this role, you will need:
One year of clinical experience.
Current RN licensure in the state of practice and one year of clinical experience.
Current CPR Certification.
Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
