Position Summary:
The Harm Reduction Clinic Nurse Care Manager (HRC NCM) is an integral member of the multidisciplinary Office-based Addiction Treatment (OBAT) team within Brockton Neighborhood Health Center's Harm Reduction Clinic (HRC).
Key duties of the HRC NCM include ongoing assessment, intakes, evaluations, triage, teaching, inductions, and stabilizations for patients with opioid and/or alcohol use disorder that are interested in treatment with buprenorphine or naltrexone, and patients with stimulant use disorders seeking Contingency Management treatment services.
The HRC NCM works collaboratively with a team of behavioral health clinicians, recovery coaches, community health workers, and providers to coordinate and manage patient care.
It is also essential that the HRC NCM coordinate patient care with external community agencies including local hospitals, treatment programs, mental health providers, and other necessary service providers.
This position requires a Registered Nurse skilled in trauma-informed care, and who is able to deliver services within a harm reduction model.
Principle Duties and Responsibilities:
Conducting initial patient assessment and intake for treatment with buprenorphine or naltrexone, including social, medical, mental health, substance use, and medication history
Assess and monitor patients in the induction, stabilization, and maintenance phases of treatment
Administer extended-release intramuscular naltrexone and subcutaneous buprenorphine to HRC patients
Recognize substance use disorder as a chronic, relapsing condition, and continue support of patients through periods of resumed use
Provide ongoing care management, education, and support for patients in all phases of treatment, including:
nurse visits, urine toxicology screens, routine labs as needed, medication teaching, monitoring, medication counts, and medication refills
Follow federal and state guidelines in providing care to opioid and/or alcohol dependent patients in collaboration with licensed prescribing provider
Complete timely and appropriate documentation and record keeping in the electronic health record
Contribute to required State OBAT reporting and paperwork
Maintain a professional environment in a multi-provider, multidisciplinary organization.
Provision of care and advocate for patients of all ages and their families based on prudent nursing judgment and acceptable standards of practice.
Perform tasks and procedures within the scope of a registered nurse (RN) and legal guidelines of the Nurse Practice Act.
Educate patients, their family members, or other supportive individuals about buprenorphine or naltrexone therapy as well as risks, benefits, potential side effects, interactions, program requirements, consents, and treatment contracts.
Involve patients in the development of the treatment plan, and working with the patients' needs
Enhance treatment readiness, supports treatment completion, ensures safety, and promotes sustained recovery outcomes for individuals with substance use disorders
Ability to practice trauma-informed care and apply de-escalation techniques
Identify community resources and promote access or referral to them
Assist patients in accessing care when a higher or lower level of care than the HRC can provide requested by the patient, or is clinically necessary
Assist patients in accessing other treatment options as needed (e.g.
day treatment, residential, outpatient, methadone detoxification)
Maintain patient confidentiality in alignment with HIPAA and 42CFR Part 2
Return phone calls, emails and other forms of communication in regards to program and routing questions to appropriate prescriber as needed.
Qualifications:
Minimum Skills, Experience, and Educational Requirements:
BSN preferred, may substitute BSN for 1 year of experience
2-3 years of experience working with homeless or underserved adult populations, community health center experience preferred
1+ years of experience in substance use disorder treatment, harm reduction, or behavioral health
Excellent customer service skills
Excellent verbal and written communications skills
Proficiency with electronic health record
Strong computer literacy, including Microsoft Word, Outlook, Excel, & PowerPoint
Ability to effectively work within both a team setting and independently
Ability to work effectively with economically, racially, and linguistically diverse customers; multilingual candidates strongly preferred
Required Certifications and Licenses:
Current Massachusetts Registered Nurse
CARN preferred, must obtain CARN within 2 years of employment
OBAT training preferred, must attend within 1 year of employment
WORKING CONDITIONS:
Works in a busy community health center in a downtown business district.
Long hours are sometimes required.
Some in-state travel required.
May require night or weekend work at discretion of direct supervisor or Director of Substance Use Services.
PHYSICAL REQUIREMENTS:
Physical demands requiring lifting of small pieces of equipment, boxes, and/or patients not to exceed twenty (20) pounds.
Mostly standing and walking required.
Visual acuity sufficient for frequent reading.
Hearing acuity sufficient for holding conversation with or without audio devices.
