The Service Coordinator provides mediation and advocacy to families of infant and toddlers with disabilities and assists them in obtaining, as well as coordinating and monitoring quality services in accordance with Community Options Inc.'s (COI) value :
Bachelor's level degree of education or four years of experience in the field of developmental disabilities or some combination of appropriate education and experience equal to four years of experience required.
Valid Colorado driver's license.
A driving record acceptable to COI's insurance company.
Acceptable criminal background check.Spanish Speaking a plus.ESSENTIAL DUTIES AND RESPONSIBILITIES:
Must meet personnel the following standards within required timelines:
Complete service coordination online orientation training modules within one (1) month of employment as a service
introductory training on the service coordination core competency requirements and the development and implementation of an Individualized Family Service Plan (IFSP) within one hundred and twenty (120) calendar days of employment.Document all completed training in the Early Intervention Provider
a parent of his or her rights and procedural safeguards, and how to exercise them.
Ensure that required information for each child referred for early intervention services is provided for entry into the statewide database in accordance with reporting requirements of the
with local administrative units and other appropriate providers to ensure the completion and compliance of a child's evaluation and assessment.Facilitate and participate in the development, review and evaluation of Individualized Family Service Plans (IFSP).Make referrals to providers for early intervention services authorized in an Individualized Family Service Plan, assist with scheduling appointments, ensure initiation within twenty-eight (28) calendar days of written parent consent for early intervention services, and coordinate, facilitate and monitor the delivery of early intervention services.Ensure that a parent is informed of the coordinated system of payment funding hierarchy and no-cost protections for families and ensure appropriate use of all available funding for early intervention services.
Coordinate the provision of medical and other services (e.g.
educational and social) the child or family needs or is receiving through other sources.
Inform a parent of available advocacy services.Facilitate development of the transition to preschool special education services or other services for a toddler approaching three (3) years of age.Assist a parent with dispute resolution regarding early intervention services, as needed.Maintain at least monthly contact with a parent whose child is enrolled in early intervention services, including written, electronic, or phone communication, and shall document the contact in the child's record.

To be flexible and able to support people during changing and stressful conditions.Demonstrate ability to adapt readily and effectively to change.Attend required staff meetings, IFSP, interagency meetings, in-services and retreats when offered.
Must have computer skills sufficient to learn state software systems.Proficient knowledge of MS Office software - especially Excel & WordPossess effective typing and writing abilities.Must be willing to travel in state on a regular basis.
Mostly day trips with minimal overnight trips.
