Are you a dedicated and compassionate Physical Therapist with a passion for helping patients regain their independence? We are looking for an exceptional Home Care Physical Therapist to join our team and make a real difference in the lives of those recovering from illness and injury.

Work primarily weekdays with a weekend rotation monthly. Enjoy the autonomy of managing your daily work schedule

About the Role:

As a Home Care Physical Therapist, you will:

Provide both direct and indirect patient care to facilitate the rehabilitation and restoration of normal body functions.
Implement personalized treatment programs based on each patient's plan of care.
Administer therapeutic modalities and train patients in therapeutic exercises, transfers, and gait.
Supervise and guide PT Assistants and PT Aides to ensure the highest quality of care.

Responsibilities:

Physical Therapy Assessment and Skilled Treatments.
Client Care Coordination.
Teaching, Supervision and Counseling Clients and Caregivers.
Documentation of Interdisciplinary Communication Supervision of Ancillary staff.
Problem Solving and Critical Thinking Skills.
Documentation.
Ability to effectively and safely transfer patients with significant physical and/or mental impairment.
Time Management Development of Plan of Care

Qualifications:

* Proof Of Current/Valid Car Insurance
• Knowledge And Experience With Electronic Health Records
• Experience With Excel, Power Point, Word, Visio, Etc.
• Valid DL With No More Than 3 Moving Violations

*Ability to assist in patient lifts, transfers, and guarded functional activity training.

Competencies Skills:

* Clear Communication Skills Both Written And Verbal
• Able To Keep Confidential Information Regarding Patients, Team Members
• Able To Withstand Crisis Situations
• Has Skills To Provides Customer Service To Patients, Team Members And Visitors

Credentials:

* PT - Physical Therapist
• BLS - Basic Life Saving certification

Education:

Minimum required: Bachelor's

Preferred/desired: Master's degree or doctorate

USD $38.81/Hr.
