Nurse Practitioner

MPA are retained by a leading HIV / AIDs care provider to recruit for a Nurse Practitioner to join their growing team in Vancouver. The client is a leading organization dedicated to providing comprehensive care to individuals living with HIV/AIDS who face additional challenges like poverty, homelessness, mental health issues, and substance use disorders.

Their Key Programs and Services Include:

Day Health Program:
Engage clients in therapeutic activities, counseling, and nursing care while providing nutritious daily meals.
24-Hour Specialized Nursing Care Residence:
Deliver personalized care, including stabilization, convalescent, and palliative services in a supportive environment.
Enhanced Supportive Housing:
Assist residents transitioning to independent living while continuing to provide essential support services.

In this role, you will be responsible for comprehensive assessment, treatment, and primary care collaborative model for participants experiencing HIV, Hep C and Substance Use Disorder. You will work in our Opioid Agonist Therapy (OAT) and Medication Adherence Program (MAP), and comprehensive assessment and case review in Residence. The Nurse Practitioner works independently and as part of an inter-professional team of Nurses, Dietitians, Counsellors, Therapists, Pharmacist and Consulting Physicians.

Responsibilities:

Diagnose and treat previously undiagnosed participants for undifferentiated disease and conditions within NPs' scope of practice
In collaboration with primary care physician, monitors ongoing care, orders appropriate screening diagnostic investigations; interprets reports of investigations and analyzes information to monitor progress.
Initiate treatment including health care management, therapeutic interventions and prescribe medications.
Provide professional mentorship to other health providers and practice independently within the interdisciplinary team.
Participate in the development of policies and procedures and protocols relevant to nursing, interdisciplinary care and health system practices.
Participate in quality improvement processes
Responsible for own professional development and maintaining competence in the position.

Requirements

Current registration as a Nurse Practitioner with BCCNM.
Recent and relevant primary care experience treating adults
Preferred experience in following areas; HIV, Hep C, Substance use disorder and end of life care.
Ability to work in fast paced, changing environments
Ability to communicate public education on health finding, diagnosis, treatment and self care.
Ability to plan and implement strategies for population-based prevention and health promotion

Salary range between $74.48 - $81.93 per hour
