Explanation:

Infection of the meninges is called meningitis. Usually it is the pia and the arachnoid that are infected. The inflammation stretches the dura and stimulates the free nerve endings in it. This is perceived as pain, which is exacerbated by neck flexion that puts additional traction on the dura. The dura is reflected and you can see the pus in the subarachnoid space.
