Responsibilities:

Communicate with customers, colleagues, and other individuals to answer questions, disseminate or explain information, and address complaints/concerns.
Coordinate team meetings and facilitate every 30 days. Document and maintain all information regarding the plan of care.
Develop ongoing working plan for team to implement. Meet with team members to ensure progress.
Ensure all members of the team receive copies of the plan of care, including the youth involved.
Educate all members and new members of the wraparound process and maintain team cohesiveness.
Ensures that individual is seen by psychiatrist, therapist, and have all needed supports with other providers.
Educate on wellness, recovery, and resiliency.
Connect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral.
Complete documentation within established time frames.
Works closely with local MAP team for flexible funding.
Meet with clients and or team members a minimum of 3 times a week depending on need.
Attend and participate bi-weekly with Treatment Team meetings.
Serve as a liaison with other professionals and agencies as related to the client
Actively recruit new referrals to ensure that your caseload stays full.
Provide 24/7 on-call support for clients as per program requirements.
Qualifications:

Bachelor's degree in human services discipline, including, but not limited to:

Social Work, Psychology, Sociology, Counseling,) and experience working with children and families in community support service position.
Upon hire, must attend 3 training courses via WRAPAROUND institute to be a certified Wraparound Facilitator, must begin credentialing process through DMH as PCSS and upon passing a test to become fully credentialed.
