Work Schedule:

100% FTE, days. Monday - Friday, eight-hour days between the hours of 8:00 am and 5:00 pm. No weekends, holidays, or on-call. You will work at East Madison Hospital in Madison, WI.

As an Advanced Practice Provider, you'll play an integral role in our inpatient and acute/emergency medicine care teams by providing high-quality, cost-effective care to patients.

We are seeking a Nurse Practitioner or Physician Assistant to:

Provide complex symptom management recommendations.
Work in collaboration with the Palliative Care Medical Director, attending medical team, and Palliative Care Team members to deliver palliative care to patients in the acute care setting with life-limiting chronic diseases.
Effectively communicate with patients and family by utilizing concepts of motivational interviewing.
Perform assessment process to include patient history, physical, psychosocial, and spiritual assessment.
Provide support, counseling, and information to patients and families dealing with end-of-life issues or with issues related to life-threatening or serious and complex illness.
Facilitate patient/family conferences as related to palliative care or hospice.

Education:If Nurse Practitioner: Master's or Doctorate's degree from accredited Nursing Program.
If Physician Assistant: Bachelor's or Master's degree from accredited PA Program.

Licenses and Certifications:
Minimum - State of Wisconsin NP or PA licensure
If Nurse Practitioner*:

Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND
Active board certification as a NP in area of clinical practice. These may include the following: Adult; Adult/Gerontology Acute Care; Adult/Gerontology Primary Care

If Physician Assistant*:

Active PA license, including prescriptive authority, in state(s) of practice AND
Active PA board certification

All Nurse Practitioner and Physician Assistant applicants must have*: Active DEA license or ability to apply for such license prior to or upon hire
