Schedule: Flexible, to include Saturday and/or Sunday 8:00AM-4:00PM, occasional on-call hours
Starting Pay: $50 hourly, plus $8.00 weekend shift differential

Who We're Seeking:

Join our team as a Per Diem Weekend Home Care RN to work with patients in their home environments.
Your Responsibilities:

As a member of our dedicated team, you'll deliver high quality care directly to individuals within the comfort of their homes in a designated geographic territory. You will collaborate with colleagues, local physicians, and community providers to provide exceptional health care to our clients.

Requirements:

VT Licensed RN (BSN preferred)
Home care experience and/or previous case management experience in a home care setting and discharge planning and/or utilization review experience preferred.
Valid driver's license
