The Clinic Patient Management RN II is a Full Time, Regular position working Monday-Friday day shift at Holy Family Outpatient Based Imaging Center. NO WEEKENDS OR CALL. SIGN ON BONUS AVAILABLE FOR EXPERIENCED NURSESGeneral Description: The Clinic Patient Manager RN II must be able to work effectively and efficiently in a fast-paced environment in meeting continual deadlines; handle multiple priorities with constant interruptions; be highly organized to meet deadlines. Interacts with physicians, other departments within Inland Imaging, patients and family members and outside contacts. Excellent written, verbal, and personal communication skills; tactful and diplomatic; and be highly confidential. Makes decisions regarding daily work priorities and other decisions based on company policies and procedures

This position is primarily a nursing position in the Interventional Radiology Clinic.

  • Provides continuity of care guidance for patients with chronic diseases and/or complex disease states.
  • Uses knowledge of illness, diagnosis, and treatments in comprehensive, holistic assessments of patients.
  • Provides teaching to patients/families related to patient's diagnosis, pathology, medical and nursing treatment plans, discharge needs, and health goals; documents each element of care per encounter in compliance with clinic standards.
  • Utilizes appropriate resources to gather data and assess patient's actual and potential needs and identifying potential education and treatments necessary to meet these needs. Plans for and manages interventions and treatments appropriate to patients' medical and nursing diagnoses, age, abilities, and resources. Communicates this information regularly to clinic staff and providers as appropriate.
  • Maintains complete documentation of all critical and essential aspects of medical history and information in the patient's file. Manages patient related activities in an outpatient, ambulatory clinic, and/or inpatient, acute care setting.
  • Mobilizes resources and manages the system in response to patient need according to interdisciplinary plan of care.
  • Checks charts/EPIC and confirms all necessary records and imaging reports are received prior to appointment.
  • Delivers education and instructions for patients undergoing treatments and interventions.
  • Performs follow up calls to patient after all hospital and clinic procedures. Triages post-op questions/concerns.
  • Places imaging/lab orders and medication prescriptions for physicians and ARNPs.
  • Develops, updates, and implements patient instructions and clinic protocols.
  • Assists with follow up care and data collection for patients enrolled in clinical trials.
  • Works with MA to room patients, take vitals, review medications and complete ROS as needed
  • Provides a high level of customer service by maintaining a positive, open and honest interpersonal relationships with customers such as physicians, co-workers, visitors, patients, and referring offices.
  • Promotes and demonstrates effective, open and honest written and verbal communication with co-workers and supervisors.
  • Demonstrates self-management behaviors, following established policies and procedures and promote an efficient, cost-effective work environment.
  • Demonstrates flexibility by readily adapting to different situations to complete work in a timely manner.
  • Practices strict HIPAA and general confidentiality for all patient and study data.
  • Performs other duties as assigned.
General Duties/Responsibilities:
  • Ability to maintain strict confidentiality within the Inland Imaging companies and Inland's customers.
  • Follows all Health and Safety policies and guidelines of Inland Imaging or its partners depending on work location.

  • Valid state driver's license and proof of insurance.

  • Detail oriented; ability to multi-task; organized and able to work in a fast-paced environment.
  • Demonstrates self-directed learning and participation in continuing education through professional journals, approved seminars, etc.
  • Ability to consider individual needs in communication with and assessment and treatment of patients of all ages.
  • Adheres to departmental standards and personnel policies by demonstrating professional demeanor in conduct and appearance.
  • Follows company departmental standards and personnel policies by using good teamwork and communication skills to help identify concerns and solutions, assisting where needed to ensure a smooth functioning department.
  • Performs other duties as required by displaying team spirit and self-growth, accepting and performing other projects and responsibilities, and requesting other projects and responsibilities.
  • Attendance is required for this position.
  • Rotates to other shifts and locations as needed.
Supervisory Responsibilities:
  • Acts as mentor and advisor for clinic Medical Assistant and Receptionist. Works closely with IR clinic APPs to manage all aspects of patient care and follow-up.

Qualifications:
  • Education:
    • High School Diploma/GED required
    • Accredited Nursing Program
    • Bachelor's Degree or higher desired
  • Experience:
    • One to two years' experience as a Registered Nurse required
    • Excellent patient assessment skills
    • Interventional radiology experience - helpful
    • Anatomy and medical terminology required.
    • Medical office experience.
    • Understanding of procedures to be read by radiologist preferred.
  • Licensure: Must maintain current Washington State RN licensure
  • Certifications: Current CPR/BLS required
  • Computer Skills:
    • Experience with computer applications and keyboarding - required
    • Experience with the following computer applications is highly desired: Outlook; Microsoft Suite, Workday, Qgenda, EPIC; Imagecast; IDXterm; MedInformati.
