*Description
• Responsible for providing case coordination and a full range of social work services for non-medical issues, including psychosocial assessment, treatment planning, utilization management, therapeutic interventions, discharge planning, crisis intervention, referral and resource, and acts as a liaison between USFHP Medical Director, Director of Operations, Case Managers, members and Medical providers including Skilled Nursing Attending.

• Required qualifications:
• Bachelor's Degree in Social Work, from an accredited school. Training in using efficient, focused treatment approaches to patient problems.
• 5 years of Relevant to job.
• Experience in using efficient, focused treatment approaches to patient problems. in the provision of outpatient community services and discharge planning.
• Preferred qualifications:
• Master's Degree in Social Work, from an accredited school.
• Upon hire: Licensed, registered, or certified by the State of Washington as required for that discipline

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
