Join WCS Healthcare Partners as a Skilled Case Manager We are seeking an experienced and compassionate CASE MANAGER to join our team at WCS Healthcare Partners in Washington Heights, New York.
About the Role:
This is a 13-week contract position with 36 hours per week, offering a unique opportunity to make a positive impact on the lives of young adults.
About the Job Function:
As a Case Manager, you will provide integrated counseling and case management services that effectively engage residents and promote health and wellness, personal growth, and housing stability.
Your responsibilities will include collaborating with residents to develop client-centered service plans, utilizing a harm reduction approach to support clients around issues of mental health and substance use, making referrals to various care coordination agencies, and actively engaging residents through regular visits, community outings, and escorts to appointments as needed.
About the Requirements:
A Bachelor's degree in a related field such as social work, sociology, or psychology is required, with a minimum of one year of experience working with single adults impacted by poverty, chronic homelessness, mental illness, substance abuse, and medical issues.
Highly organized and able to manage a diverse caseload of clients with diverse needs.
Able to work well under pressure, including in crisis situations, and be flexible with work schedules when necessary.
Track record of proactive, strength-based work with individuals and families.
Experience with electronic case management systems and evidence-based models, specifically Motivational Interviewing, is preferred.
