Pay Rate: $2252.92 - $2468.62
PT- Home health needed asap for 13 weeks
Tuesday - Saturday. First week will be M-F to facilitate orientation
Physical Therapist needed to provide one-on-one therapy care for patients in their home setting!
We are looking for a candidate with no requested time off.
Primary Job Responsibilities:
Assess a person's condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence.
Collaborate with interdisciplinary team and colleagues to ensure a person's needs are met.
Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests.
Utilize a variety of therapeutic modalities to maximize care provided to clients
Mileage Details: IRS rate, applied for travel between patients, not for commute
Requirements:
Vermont PT licensure
ACLS
BLS/BCLS
Home Health experience and OASIS experience a must as this candidate will case manage
McKesson/Netsmart EMR experience strongly preferred
1 year experience

Certifications: BLS-AHA(Copy needed); ACLS-AHA(Copy needed)
On Call: $2- hr
