The ideal candidate will be results-driven, team-and detail-oriented professional who is comfortable working on multiple tasks in a deadline-oriented environment.
Responsibilities and Duties:
Provide assessment, coordination, advocacy, and support to the clients of the CSP program for the purpose of creation, development, or enhancement of an individual support network.
Provide internal and external training related to the value and necessity of support networks in the maintenance of positive mental health.
Assess the volume, reciprocity, and positive value of individual support networks.
Develop and implement treatment plans and interventions aimed at improving the support networks of assigned caseload or individual.
Develop contacts and relationships with collateral community agencies or organizations that may assist in the fulfillment of support network functions.
Teach basic communication skills that would enable an individual to succeed in developing positive relationships with others.
Produce billing forms, logs, data collection, and clinical documentation in accordance with Organization standards and State and Federal licensing regulations.
Qualifications and Skills:
Master's degree in Rehabilitation or related field or certificate as a career development facilitator, or
A Bachelor's degree in the human services, business, or related field
Valid driver's license required
Supported Employment Professional Certification preferred, or working toward
