Weekend aid visits and PRN M-F AM/PM
We are seeking an outstanding Hospice Aide to join our team at Ovation Hospice. Come join a team of dedicated, smart, and caring professionals as they work together to make sure our patients come first, always.
Role Overview
The hospice aide is responsible for providing personal care under the direction of a registered nurse. We have wide scope of patients from in home, assisted livings and SNF's. Evening shift is often from 5-10 PM. Weekend mornings start around 7 am until required visits are completed.

Responsibilities
Document care provided in timely and professional manner.
Report any deviations from the Plan of Care to the Case Manager
Hands on assistance with ADL's depicted in patients plan of care. (Incontinent care, oral hygiene, bathing assistance, feeding exct)
Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care any by active participation in team and staff conferences.
Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. Reports any changes/areas of concern to Case Manager.
Assist with ambulation, transfer, range of motion if documented on the Hospice Aide Plan of Care.
Participate in QAPI activities.
Qualifications
Due to state regulations we require active CNA license.
One year experience as a CNA, hospice aide or home health aide.
At least 18 years old.
Must be a licensed driver with an insured automobile in good working order.
The ability to communicate well, both verbally and in writing.
Ability to work as a team and also independently
Meet training requirements by the state of Utah.
