Summary:
Must be AHA BLS certified, current license, and primary source verification within 30 days prior to start of assignment. Ascension Color Vision needs to be completed prior to start and updated every 3 years.
Required Certifications:
• BLS Certification
• Ascension Color Vision Test
• Core Competencies (HIPAA, Joint Commission -NPSG, OSHA Topics)
• Ascension EKG Exam (90% or higher with only 2 attempts)
Required Documents:
• Driver's License/State ID for proof of primary residency
• Badge Photo with a white or blue background
• Statement of good health or completed physical exam prior to first assignment, updated annually thereafter
• 2 Step PPD OR IGRA (Quantiferon/Tspot) testing protocol
• Evidence of immunity, vaccination or titer results for Varicella
• Evidence of immunity, vaccination or titer results for MMR
• Tdap vaccination valid for 10 years
• 12 Panel Drug Screen within 30 days prior to start and updated annually
• Ascension Education Verification with no gaps
• Confirmation of Assignment to be uploaded by the agency
• Influenza Vaccine Exemption/Declination
