Job Summary

Assists patients and families in identifying bio-psychosocial needs through assessments, screenings, and surveys. Addresses bio-psychosocial needs by providing emotional support and education, as well as determining appropriate resources and making referrals, all the while advocating for patients' rights and wishes.

Essential Duties and Responsibilities

• Conducts initial screening and evaluation of patient and families.

• Completes a comprehensive psychosocial assessment of patients.

• Performs assessments, screenings, and surveys throughout the provision of patients' care to identify needs, providing follow up with intervention and referrals.

• Documents and maintains as appropriate in the patient's chart.

• Helps patients and families understand the illness and treatment options, as well as consequences of various treatments or treatment refusal.

• Supports patients and families adjust to circumstantial change, possible role modifications, and exploring emotional and social responses.

• Educates patients on the roles of the interdisciplinary team members, assists patients and families in communicating with one another and to members of the interdisciplinary team, and interprets information.

• Educates patients on the levels of care, entitlements, community resources, and advance directives.

• Employs crisis intervention.

• Recognizes underlying mental illness, provides or makes referrals for individual, family, and group psychotherapy and medication management.

• Educates interdisciplinary team members on patient psychosocial issues.

• Promotes communication and collaboration among interdisciplinary team members.

• Coordinates patient discharge and continuity of care planning.

• Promotes patient navigation services.

• Identifies resources and funds to finance medications, durable medical equipment, and other needed services.

• Communicates and ensure understanding about follow-up care among patient, family and interdisciplinary team members.

• Advocates for patient and family needs in different settings: inpatient, outpatient, home, and in the community.

• Promotes rights of patients through advocacy.

• Assists in developing, implementing and facilitating new programs, groups and events related to social services.

• Participates in the development or revision of policies and procedures pertaining to social services.

• Serves as an advocate in the community to promote social services available within the hospital.

• Works willingly and accepts assignments as given.

• Attends all mandatory meetings and programs.

• Participates in opportunities that enhance personal and professional growth.

• Performs other duties as necessary or assigned.

• Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct.

• Complies with all applicable state and federal regulations.

• Maintains a safe work environment using safe work practices, utilizing standard precautions and complying with hazardous waste procedures at all times as described in St. Anthony's Safety Manual.

• Maintains the confidentiality of patient, resident, employee and organizational information.

Supervisory Responsibilities

Supervisory responsibilities may include overseeing the work of bachelor level social workers within the organization as directed by the Department Vice President and/or Lead Social Worker.

Qualifications

Education and/or Experience

• Master's degree in social work.

• Holds a current state license as a Social Worker, preferred.

• Experience in a health care setting preferred.

Position Requirements, Licenses, Certifications, Registrations

• Basic Life Support certification.

• Reliable transportation, current valid Iowa driver's license and proof of automobile insurance coverage.

• Good interpersonal and written and verbal communication skills.

• Physical, sensory, and cognitive abilities sufficient to perform essential functions.

Language Skills

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or government regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from managers, customers, patients, families and the general public.

The employee must frequently lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, color vision, depth perception and ability to adjust focus. May be exposed to infectious diseases and occupational dangers like accidental needle pricks. Must be able to withstand physical and emotional stress.
