About the Role
This is a unique opportunity to join our team as a Hospice Care Nurse Specialist. In this role, you will utilize your expertise in nursing to provide high-quality care to patients with terminal illnesses. You will work closely with the interdisciplinary team to develop individualized plans of care that meet the unique needs of each patient.
Responsibilities
Your responsibilities will include:
Assessing patient needs and developing individualized plans of care;
Providing education and support to patients and their families;
Collaborating with the interdisciplinary team to ensure seamless care transitions;
Maintaining accurate and timely documentation of patient information;
Participating in bereavement follow-up and support programs;
Facilitating continuity of care through effective communication with other team members and agencies;
Communicating verbally and in writing with pharmacy, DME vendors, material management, and insurance companies to help control agency costs.
Requirements
To be successful in this role, you will need:
A Associate's Degree- Nursing or higher;
No experience required, but 2 years of Acute Care Nursing experience is preferred;
RN - REGISTERED NURSE certification;
A valid driver's license and an acceptable motor vehicle record;
A certificate of auto insurance.
Benefits
We offer a competitive salary range of $65,000 - $85,000 per year , depending on experience, plus benefits including health, dental, and vision insurance, 401(k) plan, paid time off, and opportunities for professional growth and development.
