We are seeking a RN Field Supervisor in the Fond Du Lac area.
The RN Field Supervisor is responsible for the coordination of high quality, cost-effective patient care.
This position is responsible for the clinical operations and work flow of the agency including performance of and/or oversight of crucial activities including scheduling, staffing, intake, physician communications, billing and reimbursement process, and overall compliance.

Manages, supervises and ensures high quality cost-effective patient care and services.
This includes overseeing:
referral coordination, assuring that patient needs are continually assessed and coordinated, and assuring the development, implementation, adherence and updates of the patient's individualized plan of care.
Ensures coordination with physician, family, and other clinicians involved in care as appropriate.

Communicates effectively with staff to ensure they are informed and educated through staff orientation, case conferences, mandatory in-services, including but not limited to monthly staff/safety meetings.
Ensures that only qualified personnel are hired and competent to perform all services and tasks assigned, including ensure licensure and certification requirements are met.
Provides in home visits as needed.
Provides oversight and ensures accurate completion of clinical medical record documentation, including electronic visit verification.
Assists with complaint investigation, resolution, and follow-up as assigned.
Provides oversight to the agency's infection control plan and practice.
Serves as the Emergency Management Coordinator in the absence of the Executive Director.
Participates in the organization's Performance Improvement and Compliance Programs including adherence to policy and procedures and the development, implementation, and completion of action plans.
All other duties assigned.

Must be a Registered Nurse licensed in the state of practice.
Minimum of one (1) year of Home Care related experience preferred.
Supervisory or management experience preferred.
Current CPR Current Driver's License and vehicle insurance, and access to a dependable vehicle, or public

transportation.

State Specific Requirements:
For Category I and II Medicare Certified Home Health:
CT:
To be appointed Supervisor of Clinical Services, must have one (1)year fulltime clinical experience in a home health agency and one of the following:
(1) master's degree or (2) bachelor degree with a minimum of three (3) years full-time clinical experience or (3) associate degree with a minimum of three (3)years full-time clinical experience in past five (5) years and evidence of certification by ANA (What is ANA?) as a community health nurse or completion of at least six (6) credits received within two (2) years in community health nursing theory or six (6) credits in health care management from an accredited college or university program or school of nursing.

LA:
One year of clinical experience as a registered nurse.
KY, NY, OH, TN, WI:

No additional requirements.

For Category III and IV:
FL:
Must have at least one (1) year of supervisory experience as a registered nurse.
TX:
Must be a registered nurse (RN) licensed in Texas and have at least one (1) year experience as

an RN within the last 36 months.
AR, CT, PA, GA, MO, NC, NY, PA, TN, VA:

Skill Requirements:
Prior management experience preferred.
