Overview

The RN Coordinator is responsible for the coordination of clinical activities of the Wound Care Center. This may include patient care assessment, patient care planning and implementation, and working on patient care quality improvement. This position works in collaboration with the Program Director and the Medical Director to meet the Center's program objectives and to help ensure optimal healing outcomes and patient satisfaction.

Responsibilities

Clinical Excellence/Patient Care

Assesses the needs of patients and families, providing appropriate teaching and making the necessary referrals to provide continuity of care.

Develops individual plan of care using a collaborative approach with the patient/family and health care team.

Promptly completes documentation to assure an accurate legal record of patient's care.

Delivers high-quality care utilizing the nursing process while demonstrating knowledge and clinical skills of the specialty required on assigned unit in accordance with established policies, procedures and standards of care.

Utilizes holistic approach (considering biopsychosocial, spiritual, educational, safety, and related criteria) appropriate to the age of the patients and families served in the assigned service area.

Delegates and provides direction for other healthcare providers and holds them accountable for their involvement in the plan of care.

Utilizes critical thinking in making independent judgments and maintains responsibility and accountability for the knowledge of conditions of assigned patients.

Coordination

Provides case management as needed to a group of patients providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management.

Performs HBO (Hyperbaric Oxygen)-related duties as required, upon completion of an approved Introduction to Hyperbarics 40 hr. course.

Organizes and prioritizes clinical responsibilities, provides clinical oversight to clinical staff, such as Registered Nurses (RNs); Licensed Practical Nurses (LPNs) or Licensed Vocational Nurses (LVNs); Certified Medical Assistants (CMAs); Hyperbaric Oxygen Technicians, etc.

Collaborates with other health care providers, Wound Care Center Providers, Program Director, and Medical Director regarding clinical and patient needs.

Consistently looks at current processes and procedures, and identifies and implements areas of improvement, while complying with Health Insurance Portability and Accountability Act (HIPAA), the Joint Commission, the Department of Health (DOH), as well as other hospital requirements.

Engages in staff development, providing general in-service training as well as individual instruction and orientation. Provides staff recognition through positive reinforcement and constructive feedback.

Collaborates with Program Director in coordination of program objectives, from a budgetary and financial perspective.

Collaborates with Director and Medical Director to achieve Wound Care Center quality indicators.

Development and Patient Education

Continues professional development and involvement in professional activities that enhance nursing practice by sharing knowledge through networking and research.

Assesses and chooses teaching strategies appropriate to the patient's and their families' age-specific needs, culture, gender, readiness to learn and level of comprehension.

Consults appropriate resources within the hospital to provide specialized education.

Demonstrates knowledge that patient and workplace safety is a priority by preventing errors, accidents and infections and reporting potential or actual concerns immediately.

Qualifications

Minimum Requirements

Education:
• Graduate of accredited nursing program (Must obtain BSN within 5 years of hire. )
• Bachelor of Science in Nursing (BSN)

Experience:
• Three (3) or more years as RN
• Two (2) years' experience in an acute care setting
• Prior supervisory experience
• Wound care certification

License(s)/Certification(s):
• Current licensure in good standing to practice as a Registered Nurse in Iowa.
• Must possess and maintain current Basic Life Support (BLS) certification
• Valid driver's license when driving any vehicle for work-related reasons.

Knowledge/Skills/Abilities:
• Demonstrated competency in BLS (Basic Support Life)
