Conduct health assessment interviews focused on wound, ostomy, and continence-related care.
Perform focused physical assessments, develop plans of care based on diagnoses and individual client needs, and recommend medications and treatments as , revise, and evaluate standards of care.
Assist in selecting supplies, technology, and equipment.Assess and document patient responses to wound care, ostomy, continence, fistula, and complex wound treatments.
Evaluate the quality and delivery of care provided by clinical staff on the unit.Work collaboratively with the healthcare team and make referrals to other disciplines as , counsel, and assist individuals and families in preventing complications related to ostomies, wounds, or continence issues.Develop, provide, and evaluate staff education programs, including new employee orientation, in-services, and continuing education programs.Provide comprehensive assessments of patients at risk for developing pressure injuries and initiate preventative measures.Escalate and communicate challenges identified, including ethical issues and conflict resolution.Collaborate with the Assistant Director of Nursing for Wound Care to recommend best practices based on assessments.Participate in Unit/Department Quality Improvement and hospital-wide QI activities, such as incidence and prevalence studies and nursing quality
culturally competent education to patients, families, and staff, consistent with cultural, ethnic, religious, and other belief systems.Maintain the security and confidentiality of data and information in accordance with patient rights policies.Achieve and maintain professional competency by engaging in activities consistent with clinical practice and population needs.Identify available community resources for clients and assist them in utilizing these resources.Manage assets related to rented equipment, such as specialty beds and negative pressure devices.Skills Required:
Minimum of 2 years of med/surg experience.Experience in wound and colostomy care.Education:
Valid New York State RN License with Primary Source
BLS
in one of the following:
Certified Wound Ostomy Continence Nurse (CWOCN) by the Wound, Ostomy and Continence Nursing Certification Board (WOCNCB).Certified Wound Ostomy Nurse (CWON) by the WOCNCB.Certified Wound Care Nurse (CWCN) by the WOCNCB.Certified Wound Specialist (CWS) by the American Board of Wound Management (ABWM).Wound Care Certified Certification (WCC) by the National Alliance of Wound Care and Ostomy (NAWCO).Languages:
English (Speak, Read, Write).Hours:
40 hours per week (8 hours per day, 5 days per week).Additional Notes:
70% of the role will be office-based.Nurse may be required to travel to patients' homes in Manhattan, Brooklyn, Bronx, and Queens for wound care consults.Nurse must hold a Wound Care
average, the nurse will handle 15 to 20 consults per day.
