Summary of Position:
Organizes and administers individualized, professional patient care.
Provides nursing care as directed by the medical staff, and in accordance with the objectives, policies and procedures of Wilton Medical Arts Urgent Care and the state Nurse Practice Act.
Maintains standards of nursing care as established.
Participates in patient teaching and discharge planning.
Participates in Quality Improvement activities.
Maintains an understanding of the principles of growth and development to assess each patient's age specific needs and provides age specific treatment and care.
Assumes Resources Nurse responsibilities as need.
PRIMARY RESPONSIBILITIES:
Patient Assessments :
Incorporates appropriate assessment and intervention skills that are age specific in all interactions and delivery of nursing care.
Assesses patients' physical/psycho-social status.
Analyzes relevant data.
Integrates assessment results in care planning, goals and outcomes.
Provides for therapeutic interventions based on the continued assessment, prescribed orders and input from other care providers.
Provides for adequate pain management, manages IV therapy and administers medications according to critical elements.
Provides for patient/colleague/self safety and protection.
Communication:
Communicates with patients, colleagues, providers, and management in an accurate, considerate, and timely manner.
Applies patient specific communication skills in interactions that are culturally sensitive & appropriate to age.
Language barriers and sensory & cognitive deficit are accommodated.
Provides report that is pertinent, concise using SBAR.
Documents in a complete, timely and concise manner.
Problem Solving:
Employs critical thinking skills to achieve outcomes and solve problems.
Practices within limits of experience/capability.
Collects and integrates data from multi sources and seeks assistance/information appropriately.
Analyzes data critically in order to support clinical decision making.

Teaching :
Incorporates teaching in delivery of care to promote health and prevent disease.
Provides information to patient/family with consideration for individual needs.
Assess for barriers to learning and motivation.
Teaches patient/family to safely take and monitor effects of medications.
Prepares a patient/family for prescribed treatment/procedure/discharge.
Consults multi-disciplinary team as appropriate to enhance patient/family education.
Provides counseling for health behaviors.
Qualifications:
Graduate of an accredited school of nursing, and currently licensed to practice in New York State.
BSN preferred.
AAS prepared must complete BSN within 4 years of date of hire.
Minimum one year clinical experience in Emergency Medicine.
BLS required.
ACLS and PALS required within one year of employment.
Salary Range:
$34 - $55.81
Pay Grade:
N10
Compensation may vary based upon, but not limited to:
overall experience and qualifications, shift, and location.
