Key Responsibilities:

Transform the lives of our patients/residents while developing lasting relationships with them and their families.
Conduct thorough nursing assessments and work in collaboration with the nursing team, interdisciplinary staff, patients, and families to formulate effective care plans.
Set achievable, measurable short- and long-term goals for patients with relevant interventions to minimize rehospitalizations and regularly assess outcomes.
Administer medications and perform treatments according to physician orders.
Delegate patient care activities to nursing staff and oversee their tasks to ensure quality care delivery.
Facilitate communication of patient information with assigned team members across shifts.
Qualifications:

Must hold a degree from an accredited nursing program and possess a current nursing license per the State Board of Nursing.
CPR Certification is required.

USD $39.00 - USD $43.00 /Hr. Bonus:
