WCS Healthcare has an immediate opening for an RN Case Manager with a large Social Services Agency with needs in Suffolk County.
Job Function:

~ Day Shift
~8:30am-5pm flexible on start time
~ Assess individual's needs.
~ Build strong relationships with individuals, families, and the treatment team.
~ Create and ensure support staff stick to nursing care plans.
~ Schedule appointments for individuals promptly.
~ Teach individuals and support staff regarding nursing plans of care required.
~ Document individuals status, health and follow-ups.
~ Monitor health records such as seizure charts, bowel movement tracking, vitals, etc.
~ Manage medication storage properly, dispose of expired medications correctly, and keep the medication area clean.

Job Requirements:

~ Registered Professional Nurse with licensure from New York State Education Department
~1 year nursing experience working with Intellectual Disabled Adults or Case Management experience preferred
