Health Source Group currently has a top-tier client with a group of Long-Term Care facilities in the Forest Grove, Portland and Wilsonvile, OR area that are in need of experienced Registered Nurses. They have many openings for local contract and PRN.
Strong potential for extension, transfer or direct hire.
1 year experience as LTC / SNF Registered Nurses required, 2 years preferred.
13 Week Contract Period
Day Shifts: 6a-2:30p shift & Weekends
Pay Rate: $48/hr
Job Duties:
Nurse will conduct thorough and ongoing assessments of patients, including monitoring vital signs, assessing pain levels, and identifying changes in condition.
RN will develop and implement individualized care plans based on patients' medical history, current condition, and physician orders, ensuring holistic and patient-centered care.
RN will safely administer medications, including intravenous (IV) therapies, and monitor patients for any adverse reactions or side effects.
Registered Nurse will continuously monitor patients' status, recognizing and responding to signs of deterioration, and initiating appropriate interventions.
Provide comprehensive wound care, including dressing changes, wound assessments, and implementation of appropriate wound management strategies.
Educate patients and their families about disease processes, treatment options, post-operative care, and lifestyle modifications to promote recovery and prevent complications.
Maintain accurate and detailed documentation of patient assessments, interventions, medications, and outcomes in electronic health records (EHR) systems.
Assist in the development and implementation of discharge plans, including arranging follow-up care, coordinating home health services, and providing discharge instructions.
