• Ensure all prescreen forms are completed and attached for submission.
• Attach a resume and license verification for application consideration.
• Conduct a COVID test 72 hours prior and obtain results 48 hours before the start date.
• Complete credentialing checklist including:
• State License Verification

• County and Federal Criminal Background Check (7 years)
• Multi-State Criminal Search (7 years)
• Nationwide Sex Offender Search (7 years)
• HHS/OIG Search
• GSA Search
• Social Security Number Trace
• Complete a 2-step PPD or Quantiferon, or provide proof of positive PPD and chest X-ray
• SAM Search
• Oregon State Background Check (facility run)
• No holiday coverage required.
