Physical Therapist - SNF

Position : Physical Therapist (SNF)
Location : Modesto, CA
Pay scale : $60 to $68 hourly on W2
Shift : Days

Responsibilities:
• Conduct comprehensive assessments of patients' physical conditions and functional abilities.
• Develop and implement individualized treatment plans to enhance mobility, strength, and overall quality of life.
• Collaborate closely with a multidisciplinary team, including physicians, nurses, occupational therapists, and speech therapists.
• Educate patients and their families on exercises and techniques to support recovery and promote independence.
• Maintain detailed and timely documentation of patient progress, treatment interventions, and outcomes.
• Participate in team meetings and engage in continuing education to stay current with best practices in physical therapy.
Requirements :
• Doctorate or Master's degree in Physical Therapy from an accredited program.
• Valid California Physical Therapy license.
• Experience in a skilled nursing facility preferred, but not required.
• Excellent communication and interpersonal skills.
• Compassionate and patient-centered approach to care.
