JOB SUMMARY of ESSENTIAL FUNCTIONS :
Adheres to all hospital Policies and Procedures and Philosophy of Care. Consistently exhibits professional conduct that is a behavior-based expression of the hospital's Core Values. Provides professional physical therapy services to all age ranges under a physician's prescription to a population of both inpatients and outpatients with physical disabilities resulting from injury, disease or surgery. Outpatient services are performed in our Satellite clinic across from the hospital. Provides patient education to both patient and family members. A physical therapist will also work with the inpatient interdisciplinary team to provide care planning and discharge planning assistance. The therapist will also work with front desk staff (HUC) on ensuring compliance with all paperwork requirements to ensure proper coding and billing of services rendered. Uses an understanding of the principles of growth and development to assess each patient's age specific needs and provide age specific treatment and care.
REQUIRED EDUCATION, TRAINING AND EXPERIENCE; CERTIFICATION/LICENSURE :
Graduate of an accredited physical therapy school with current licensure to practice in the State of Washington. Previous work as a licensed physical therapist preferred. Current Basic Life Support (BLS) certification card is required.
SPECIAL EQUIPMENT, SKILLS OR OTHER REQUIREMENTS :
Ability to use skills acquired through education and experience to a diverse caseload of patients and age ranges, including pediatrics through geriatrics. Possesses an understanding of computer usage as it relates to report writing and documentation. Use of personal protective equipment as appropriate, documentation of immunization/immunity against hepatitis B, and training related to blood-borne pathogens is required.
WORK ENVIRONMENT AND HAZARDS :
Adheres to all State and Federal regulations pertaining to hazardous materials and BBP. Occasional exposure to communicable disease and hazardous substances. Risk of physical injury from manually transferring patients and operations of equipment

SPECIAL PHYSICAL DEMANDS :
Must be skilled in the proper use of body mechanics when transferring patients. Must be able to lift a minimum of 50 pounds. Walks and stands for most of the day
ESSENTIAL FUNCTIONS/TECHNICAL
Performs patient evaluations, assessing physical, social, and emotional needs; reviews patient medical record and physicians referral to determine treatment required
Interviews patients, family members, and others to obtain relevant information which may affect the treatment plan-of-care
Conducts tests and measurements of various organ systems (e. g. musculoskeletal, nervous, cardiovascular, etc. ); selects and administers appropriate assessments (standardized and non-standardized)
Assesses physical skills as they relate to educational, medical, social and psychological factors
Prepares written evaluation for patient's chart after initial evaluation, including a written plan-of-care as well as documentation of patient's stated goals (electronic documentation in EPIC)
Regularly re-assesses patient condition throughout treatment, modifying and changing treatment programs as indicated and based on individualized needs
Plans and implements individualized patient treatment programs, defining short and long term goals which are both measurable and functional; monitors progress and treatment to modify therapeutic goals
Selects treatment environments and activities according to individualized patient needs
Provides counseling and guidance to patients and family. Acts as a resource for other services which might assist the patient and/or family
Uses an understanding of the principles of growth and development to assess each patient's age specific needs and provide age specific treatment and care
Provides education, feedback and counseling to the patient and family about the patients physical condition and progress in treatment
Follows all safety precautions set forth by P. T. and Rehab Department. Communicates immediately to department manager any safety incident or potential safety incident
Works with the inpatient interdisciplinary team to provide care planning and discharge planning assistance
Provides individualized written home and supplemental programs as needed. Instructs patients and/or family in techniques for safe transfers; practices safety precautions for all patients
Adheres to all Medicare documentation provisions. Completes necessary paperwork to comply with standards and ensure proper coding and billing
Maintains appropriate ethical standards
Documents treatment provided and patient's response and progress in patients chart (electronic documentation in EPIC)
Orients patient to the unit, explains purpose of activity, encourages patient activity, observes for signs of fatigue and adjusts activity appropriately
Evaluates, recommends, fits and adjusts prosthetic and orthotic devices, wheelchairs, supportive devices, and other mobility aids
This position is covered by a collective bargaining agreement between EvergreenHealth Monroe and United Food and Commercial Workers (UFCW), Loca.
