POSITION SUMMARY
Under the supervision of the Peer Partner Director, the Peer Partner Case Manager is responsible for supporting the ongoing development and implementation of the Peer Partner Program.
The Peer Partner Case Manager conducts intake and screening, provides appropriate information, and provides appropriate materials or referrals.
The Case Manager serves as a bridge between families, Child Protective Services and local services providers offering targeted family support programming.
The Case Manager assists clients to access resources through City agencies, local business, cooperative and collaborative CBO's.
The Case Manager will outreach at resource fairs, and develops, organizes, and facilitate parent education classes and workshops.
The Case Manager will develop close working relationships with local organizations and will maintain a database of local resources.
The Case Manager works as a team member with the other Peer Partner staff to ensure a holistic approach to the family needs.
Much time and energy is spent in case management approaches.
This position is concerned with the delivery of services in the community.
To this end the Case Manager must have the interpersonal skills to interact effectively with families identified as being at risk for abuse/neglect or out of home placement as well as other community resources that may be of assistance to the families.
Case Management skills, oral communication skills with families and youth, and the ability to clearly document outcome and data per program protocols are all necessary.
The Case Manager adheres to all confidentiality procedures.
Salary Range:
$24.00 to $30.00 per hour
MINIMUM QUALIFICATIONS
21 Years old
This position requires a minimum of 1 year of experience working with the target population or a bachelor's degree in counseling, social work, or psychology.
Must have experience with the child welfare system or juvenile probation department
The incumbent must possess a variety of skills for development of case and treatment plans, and the ability to effectively case manage clients through a variety of human service.
Must be available Monday - Friday.
Ability to work occasional weekends and evenings required.
Excellent organizational skills, time management, and ability to multitask and manage multiple projects.
Strong leadership skills, program development experience, and an understanding of enhancing community

Proven ability to work with people from a variety of different ethnic, socioeconomic, educational, religious, sexual, and generational

Experience complying with confidentiality policies and procedures and mandated reporting
Ability to remain HIPAA (Health Insurance Portability and Accountability Act) compliant.
PREFERRED QUALIFICATIONS
A Bachelor's degree in counseling, social work, or psychology.
Fluency in English, Cantonese, Spanish, or Samoan
