DescriptionPosition Overview

Under direct supervision of the Health & Community Services Division Director, the Diabetes Program Coordinator will coordinate and implement the goals and objectives of the Diabetes Programs by providing diabetes prevention activities and treatment programs.

Essential Functions

Supervise the day-to-day activities and job performance of the Diabetes Staff.
Prepare and manage program budget, including oversight of purchase orders, check requests, invoices, and other financial transactions.
Develop and submit program reports to leadership positions and funding agencies.
Coordinate community-based and school-based education and prevention services and activities, utilizing both internal and external resources.
Maintain accurate client files, program files, and other necessary documents
Collect and share data in order to determine the prevalence diabetes through surveys and other statistical gathering methods.
Process applications for clients who are eligible for benefits.
Ensure individuals with diabetes receive follow-up and specialized care and assist those in identifying diabetes-related problems.
Attend training to enhance job performance.
Maintain statistics of clients served, types of services provided, and number of people requesting assistance.
Plan, develop, and prepare grant proposals to secure alternative funding to supplement program.
Develops program and policy guidelines to be submitted and approved for program service enhancement.
Actively participate and lead (when needed) in all Wellness Program activities.
Maintains confidentiality of all privileged information.
Contributes to a team effort and accomplishes related results as required.
Performs other duties as assigned and within the scope of the position.

QualificationsEducationalRequirements

Minimum: Associate degree in health-related field

Preferred: Bachelor's degree in health-related field.
• Must possess or be willing to be a Certified Personal Trainer (CPT), Diabetes Prevention Program (DPP) Lifestyle Coach, and obtain a Diabetes Community Care Coordinator Certificate within (6) months of hire*

Experience Requirements

Minimum: Two (2) years of health-related work experience.

Preferred: Four (4) years of health-related experience.

Knowledge, Skills, & Abilities

Computer:

Proficiency in Microsoft applications, including Word, Excel, Outlook, Power Point, Internet Explorer, etc.
Proficiency in the proper use of and operation of standard office equipment.

Communication:

Ability to establish and maintain professional relationships with co-workers, medical professionals, Indian Health Service, and other outside agencies.
Ability to communicate effectively with program clientele, the general public, tribal members, Pawnee Nation employees, and other concerned parties.
Must possess strong administrative, public relations, and communications skills.
Skilled in the use of business communications (proper spelling/grammar).
Skilled in the use of business arithmetic (accounting, etc.).
Ability to organize and conduct community education meetings.

Miscellaneous:

Experience managing federally funded grants and/or contracts.
Ability to pass a background screening.
Must possess the ability to provide clients with compassionate care respective of Native American customs, traditions, and beliefs.
Must demonstrate a basic understanding of clients' rights and responsibilities, including familiarity with confidentiality requirements.
Must be able to conduct research, analyze information, and interpret data.
Must possess the ability to analyze situations and adopt appropriate courses of action.
Must possess the ability to be persuasive and tactful in controversial situations.
Must possess the ability to meet deadlines, adapt to changing priorities, and work well under pressure.
Must demonstrate strong organizational skills, with the ability to prioritize.
Must possess knowledge of applicable federal, state, and local laws, regulations, and requirements.
Must possess the ability to interact and maintain good working relationships with individuals of varying social and cultural backgrounds.
Must be available to work flexible work hours, which may vary beyond the standard 8am-5pm work schedule.
Must be available for work-related travel.
Must maintain confidentiality.
Must work independently.
Must attend training, as required by the position.
Must have a valid Oklahoma drivers' license and be insurable with Pawnee Nation's insurance carrier.

Physical Demands

While performing the duties of this job, the employee regularly is required to sit; use hands to, handle or feel; reach with hands and arms; and talk or hear. The employee frequently is required to stand and walk as well as stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 25 pounds.

Work Environment

Work is generally performed in an office setting with a moderate noise level. Evening and/or weekend work may be required. Occasions may arise requiring work outdoors where exposure to natural weather conditions may occur. There is regular interaction with the public, other agencies, and employees. Tight time constraints and multiple demands from several people, departments, and/or external organizations are common. Travel may be required for training, meetings, conferences, presentations, and other events.
