Position classification
Hybrid - Job duties and expectations allow for onsite and remote work scheduled every week. Employees in this classification are regularly scheduled onsite one (1) to four (4) days per week based on the County needs and as determined by Department Director. Hybrid classified roles can be onsite more than the set minimum based on employee preference.

What Success Looks Like In This Job
The Nurse Support Program provides nurse home visitation services to families referred by Adams County Human Services. This program is designed to support families and help them stay together by providing nursing assessments, education, support and resources. The nurse utilizes family centered goals to promote self-sufficiency, healthy caregiver-child interaction and reduce further county involvement. The nurse is responsible for providing nursing assessments and interventions, and meeting program goals.
The Professional Public Health Nurse for the Nurse Support Program (NSP) is a unique, relationship-based, public health nursing role which requires the nurse to establish and maintain a trusting therapeutic relationship with each client, throughout all phases of the program. The nurse assesses the physical, emotional, social and environmental needs of the family and provides health education, referrals, and case management. This position fosters a relationship between nurse and client as a basis for proactive change. This role works with families referred to the Program by Departments of Human Services.

NSP Nurse Home Visitor PHN II: $ 76,497. 92 - $ 110,921. 98
NSP Nurse Home Visitor I (PHNI) is an entry-level position performing professional nursing process activities related to adults and children such as assessment, planning and evaluating care and treatment of adults, infants, and children. Must have completed an accredited Bachelor of Nursing degree program and has up to two years of nursing practice. The NSP NHV PHN I is learning program level processes and interdepartmental roles. The position develops therapeutic relationships, provides evidence-based education, support, and resources for the families in the community. Participates in continuous quality insurance activities, and practice improvement activities such as Practice Council. Recognizes the Public Health tenants including the public health nurse role and functions with responsibility as required by the Public Health Nursing Scope and Standards of Practice.
NSP Nurse Home Visitor II (PHN II)The Nurse Home Visitor PHN II (PHN II) is a mid-level position that demonstrates advanced nursing process skills through nursing experience or competency. Must have more than two years nursing experience. In addition to the responsibilities noted above, this position requires the NSP NHV II to have additional professional skills and experience in:
active engagement in continuous quality improvement,
active participant on agency and community committees,
competency assessment,
creative problem solving,
critical thinking,
outcomes measurement,
practice improvement leadership such as Practice Council Chair/Co-Chair,
public health and program model of care champion,
teaching, precepting and mentoring, and
training and on-boarding new PHNs.
Examples of Duties for Success
Client Relationships, Community Partner, and Case Management
Promotes the maintenance of health through teaching, conducting needs assessment and case management, direct nursing assessment, and resource coordination. This includes providing information and setting goals in prenatal and postpartum care, nutrition, mental health, parenting, child health and development, family planning, and life course development.
Prioritizes multiple work assignments, including home visits with clients, thorough and accurate and timely completion of required nursing and programmatic documentation.
Develops and maintains therapeutic relationships with a culturally diverse client population from a variety of socio-economic backgrounds, utilizing concepts of reflection and motivational interviewing; modeling client-centered, strengths-based, and solution-focused approaches.
Maintains a program-specific active case load. Continues to monitor and support client engagement in the program and proactively implements strategies to retain clients in the program.
Ensures and adheres to professional boundaries.
Collaborates with related community agencies and service providers through client referrals, written and verbal communications, and case/team conferences.
Represents Adams County as a health resource person in the community. Interprets public health programs and policies to individuals, families, community groups, schools, physicians, and other collateral community agencies and service providers.
Develops and maintains community relationships to support client referrals to needed services and referral to the NSP.
Represents the NSP vision, mission, and model in actions, and verbally to both internal/external customers and colleagues.
Participates in the promotion of public health and NSP services through community efforts, including serving on community boards and committees and presenting public health education to individuals and community groups.

Exercises independent judgment in formulating and implementing plans for nursing and general health care for individuals and families based up.
