Key Responsibilities:
Conduct nursing assessments and collaborate with the healthcare team, patients, and families to develop effective care plans.
Set realistic, measurable short- and long-term goals for patients, implementing interventions as needed to minimize rehospitalizations, and continuously evaluate outcomes.
Administer medications and perform treatments based on physician orders.
Delegate patient care tasks to team members and supervise their execution to ensure high standards are met.
Relay patient information effectively among staff and during shift transitions.
Qualifications:
Graduate of an accredited nursing program and possess a valid nursing license from the State Board of Nursing.
Current CPR Certification.
