Your Role:
Perform nursing duties in line with facility policies, state regulations, and best practices.
Collaborate with interdisciplinary team members to create and update individualized care plans.
Implement care protocols focused on falls prevention, safety, pain management, and infection control.
Assess and document residents' physical and mental status, reporting changes to the treatment team.
Lead and supervise direct caregivers, ensuring efficient care delivery.
Foster positive relationships with team members, residents, and their families for transparent communication.
Administer medications and treatments while maintaining accurate records.
Contribute to a positive and supportive work culture.
Qualifications:
A nursing degree or diploma from an accredited institution is required.
Experience in long-term care and knowledge of elderly care is preferred.
A valid RN or LPN license in New Hampshire is required. CNC certification is a plus.
Maintain CPR certification and complete annual continuing education credits.
