As a Nurse Practitioner, you will play a pivotal role in providing primary care services to patients across multiple sites, collaborating closely with our Medical Director and Director of Medical Practice Operations. This exciting role comes with a dynamic schedule across our primary care network, connecting you with our seasoned clinicians for a multidisciplinary approach. The Nurse Practitioner role is a vital position in the Monadnock Health Partners (MHP) network of primary care practices. Responsibilities:

Patient Care Responsibilities:
• Patient-Centered Care: Conduct thorough assessments and collaborate on personalized care plans.
• Comprehensive Treatment: Diagnose and treat illnesses, prescribing medications as needed.
• Preventive Healthcare: Deliver preventive care services like screenings and education.

Administrative Duties:
• Health Record Management: Maintain accurate and compliant electronic health records.
• Teamwork & Communication: Collaborate with healthcare professionals for optimal patient outcomes.
• Quality Improvement: Participate in initiatives to enhance patient care practices.
