GENERAL SUMMARY
Assists the Pharmacy Manager in establishing policies, procedures, standards, and objectives for the provision of pharmacy services, ensuring that quality and compliance standards are met. Assists with implementation and management of clinical pharmacy services. Performs all other pharmacist duties as required by departmental needs.

FUNCTIONAL DUTIES AND RESPONSIBILITIES
Clinical Duties
As dictated by departmental staffing needs, performs all central and decentral pharmacist staffing responsibilities, including verification of all medications dispensed from the pharmacy, responding to clinical inquiries from non-pharmacy staff, order verification, and clinical monitoring.
Evaluates medication orders for appropriate dosing, drug allergies, drug interactions, duplicate therapy, rational drug therapy and incompatibilities and recommends appropriate therapy changes to optimize patient care.
Conducts therapeutic drug monitoring of vancomycin, warfarin, aminoglycosides, and other narrow therapeutic index medications.
Dose adjusts medications per policy based on patient renal function.
Attends multidisciplinary rounds to participate in patient care optimization.
Conducts medication reconciliation for admitted patients.
Conducts weekly patient workup of chemotherapy regimens to evaluate for dosing accuracy, appropriate supportive care, drug interactions, and patient side effects. Makes recommendations to oncologist as needed to optimize oncology therapy.
Verifies medication orders for chemotherapy and supportive care medications to ensure accuracy and safety.
Prepares compounded sterile preparations, including chemotherapy, as needed, according to USP guidelines.
Evaluates compounded sterile preparations, including chemotherapy, for appropriate admixture, compatibility, and preparation.
Serves as a resource to providers, nurses and patients on the appropriate use of medications, drug interactions, side effects, and dosing.
Supports the pharmacy manager in implementation of clinical pharmacy programs and initiatives.
Supports the pharmacy manager in quality improvement initiatives.
Participates in and/or leads medication-related committees to accomplish departmental or organizational goals, initiatives, or regulatory requirements.
Operational Duties

Supervises, teaches and assists pharmacy technicians, pharmacy students, and pharmacists in all workflows, including tasks such as sterile compounding.
Assists with medication distribution and maintenance of automated dispensing cabinets as needed.
When necessary, supports management of pharmacy inventory, including placing replenishment orders for medications from the pharmacy wholesaler or other vendors in an accurate and cost effective manner.
Performs technician duties as needed.
Supports the manager in implementation of operational changes and initiatives.
Assists the manager in development of policies and procedures.
Complies with and supports compliance with regulatory requirements, including controlled substance handling.
Ensures the maintenance and proper operation of equipment and supplies in the pharmacy.
Directs the operations of the Pharmacy in the manager's absence.

POSITION REQUIREMENTS
Knowledge / Skills / Abilities
Ability to work independently with minimal direct supervision, with the ability to set priorities and make critical decisions.
Effective oral and written communication skills,
Strong interpersonal and team skills.

Experience
1 to 3 years experience required.
Hospital pharmacy experience preferred

Education
Graduate of an ACPE accredited School of Pharmacy.
Current NH pharmacist licensure required
