About the job:

The Registered Nurse provides general and skilled nursing care to participants, executes interventions in accordance with the plan of care, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family education and communicates participant's condition to primary caregivers, physician and care team.

Pay:
$31.39-$42.56/HRRequired Qualifications:

Graduate of a school of professional nursing. Current North Carolina Registered Nurses (R.N.) License, Cardio Pulmonary Resuscitation (CPR) certification required, Bachelors of Science Nursing (BSN) preferred.
Unencumbered license as a Registered Nurse (RN) in the State of North Carolina required.

A minimum of one year's experience in working with the frail or elderly population required.

Must have a valid driver's license, proof of insurance and have means of transportation.
Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Ability to read, speak and write fluently in English.
Broad knowledge base of physical, mental, and social needs of the frail elderly and their families

Essentials:

At the direction of the In-Home Services Supervisor, performs initial assessments of participants in their home environment to determine their individual needs and coordinates plan of care with care-giving resources. Coordinates with the Interdisciplinary Team to develop plan of care for each participant.
At the direction of the In-Home Services Supervisor, performs annual reassessments of all Senior Community Care participants to determine that appropriate in-home services are provided and that participants are in the least restrictive environment.
At the direction of the In-Home Services Supervisor, performs quarterly supervisor visits to Home Health Aides.
Provides skilled supportive and restorative nursing care to participants as ordered, re-evaluating their plan of care and health status on an on-going basis.
Maintains participants' medical record and fulfills agency charting and reporting requirements.
Re-evaluates the In-Home Service plan of care and monitors participants' health status in home.
Provides skilled supportive and restorative nursing care to participants as ordered/requested
