Job Title: Physical Therapist
Address: 1515 W Main St Suite E Molalla OR
Facility would like to offer a $#,500 completion bonus to the traveler after 13-weeks. Terms: 468 hours in a 13-week period with no more than 2 call offs or tardies. All Documentation must be completed in a timely manner and satisfactory.

Outpatient Experience is preferred.
Will accept New Grads? NO
Will accept First Time Travelers? Yes
Float to other clinics: Yes
Patient Population: all ages/general orthopedic/vestibular/post op
36 hours guaranteed with the expectation of working up to 40 hours.
OT must be approved by director before any OT hours are worked.
Productivity expectation is 60 visits/ week; Will see 12-14 patients/day; Double-booking of follow up visits expected.
