Position Summary

The H. E. A. L Team supervisor will be responsible for overseeing the duties of both inpatient and outpatient H. E. A. L. team member positions for the Life Outside of Violence (LOV) program at our partner adult Level - 1 Trauma Centers (Barnes Jewish Hospital, SSM St. Louis University Hospital). The person in this position will provide guidance to H. E. A. L. team members and oversee daily triage of eligible LOV program participants, conduct patient outreaches as needed, share information about the LOV program with community partners, and provide 1:1 consultation with H. E. A. L. team members in close coordination with the LOV program manager.

Primary Duties & Responsibilities:

Provide daily support and supervision to inpatient/outpatient H. E. A. L. team members in the hospital and community settings.

* Provide direct supervision to H. E. A. L. Team members and conduct regular individual supervision.

* Participate in training team members, providing work assignments, evaluating performance and conducting professional development plans.

* Ensure that the productivity of H. E. A. L team members meets/supports the overall operational goals as established by program leadership.

* Ensure H. E. A. L. Team members deliver outreach services in compliance with all relevant contractual and regulatory standards.

* Monitor and evaluate outreach activities to ensure quality and effectiveness.

* Communicate with community-based organizations to build synergy and expand programmatic offerings.

* Facilitate referrals to other community-based organizations and preferred partner referrals.

* Correspond with H. E. A. L. Team worker to ensure proper communication.

* Collaborate with a LOV Clinical Case Manager for purposes of enrollment and transition of therapeutic services.

* Check in with hospital social worker to ensure all patients that are eligible for LOV programming have been identified and provided with crisis support and engagement for possible program enrollment.

Completion of required administrative tasks.

* Tracking hours and mileage for H. E. A. L. Team members accurately for invoicing procedures.

* Provide regular reports on progress of H. E. A. L. Team members works to program manager.

* Maintain list of LOV eligible participants currently in outreach for the purpose of case consultation and care coordination.

* Oversee the H. E. A. L. Team Members documentation of all activity in a consistent, accurate and timely manner as required.

* Maintain or create outreach materials, talking points, and presentation materials related to outreach and engagement activities.

* Provide all required information for weekly/monthly/quarterly/annual reports.

Maintain communication with LOV team to ensure quality service.

* Participate in weekly team staff meetings.

* Participate in daily check in meetings with H. E. A. L. Team members.

* Communicate through expected documentation processes (EPIC and RedCap).

* Check email a minimum of once per shift.

* Attend and participate in all required LOV staff trainings.

* Attend and participate in professional development opportunities as identified by leadership.

Provide patients/families/communities with resources.

* Ensure H. E. A. L. team members are making appropriate community referrals based.

* Maintain relationships with other community members and organizations related to violence prevention activities and approaches.

* Provide follow-up crisis support and engagement as needed in efforts to support H. E. A. L. Team members.

* Attend and help facilitate community events related to reducing community violence.

Salary Range

$20. 57 - $30. 84 / Hourly
