Job Summary
Works collaboratively as an active member of the interprofessional team to provide patient centered care. The RN will systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care. Addresses all patients, visitors, co-workers, and care-providers with courtesy and respect. Provide patient care in the following areas: telephone triage, patient education, maintenance of the electronic medical record, scheduling, medication administration, and direct patient care.
Primary Responsibilities
Clinical Practice and Quality of Care
Executes nursing interventions by collaborating with the interprofessional team incorporating appropriate standards of care and practice and patient outcomes.
Assesses and systematically collects any data related to patient health status and works in collaboration with the care team to develop the plan of care.
Evaluates patient's response to plan of care and works in collaboration with the care team to make any necessary adjustments.
Assumes responsibility and accountability for effectively managing nursing care of individual patients.
Documents nursing assessment and interventions to reflect comprehensive and integrated approach to patient care.
Identifies learning needs of the patient, and educates by adapting to patient learning style utilizing teach back method.
Provides care that is safe, ethical, patient-centered culturally congruent and evidence-based.
Skills and Responsibilities

Triage, telephone and in-person
Review and act on abnormal test results
Initiate and monitor intravenous (IV) infusions
Assist or perform procedures (e.g. NSTs, dressing changes, subcutaneous or intra-muscular injection, etc.)
Provide patient education
Execute protocols
Assist with medication management
Complete accurate and timely documentation in the electronic health record
Assist with management of urgent and emergent patient care situations
Serve as a resource and mentor for others
Assist with the coordination of care across the continuum
Assist with the management of patients with multiple chronic health conditions, acute episodes of care, behavioral health challenges while promoting and supporting prevention and wellness
Telecommuting as needed for operational needs
Other duties as assigned

Education and/or Experience

Associate Degree in Nursing or equivalent required. Graduation from a certified RN program required.
Two (2) years relevant experience or comparable combined experience and/or education preferred.
1-2 years of nursing experience in an Psych setting preferred.
Certifications, Licenses, or Registrations

Current North Carolina nursing license or Board eligibility required.
Current CPR certification required.
Required Skills, Knowledge, and Abilities

Knowledge and experience documenting patient care in an electronic health record.
Excellent verbal and written skills
Ability to make knowledgeable clinical assessments and judgements
Ability to provide support to the members of the care team (team work)
Must possess initiative and ability to work independently
Utilize critical thinking skills in the decision making process.
Ability to maintain confidentiality of medical records
Ability to provide support with compassion and respect for the individual.
Ability to prioritize workload, exhibit initiative, and multi-task within the department and across the division is required.
Staff must demonstrate appropriate judgment in handling daily assignments.
Staff will demonstrate appropriate technical skills with the utilization of the electronic health record and office based computer systems.
Must demonstrate appropriate customer service orientation in all interactions within the organization and with patients, co-workers, care providers, and visitors.
Employee must demonstrate discretion in dealing with medical, fiscal, and personnel information as is appropriate to position.
Conversational Spanish preferred.
Physical Demands

Frequently stands, walks, uses hands to handle or feel, reaches with hands and arms, bends, talks, and listens.
Sometimes sits, climbs, and stoops or kneels. Lifts up to 50 pounds maximum with frequent lifting/and or carrying objects up to 25 pounds.
Work Environment

Potential for routine exposure to infectious diseases, hazardous chemicals, etc.
Moderate noise level typical of a busy clinical office with office/medical equipment and foot traffic.
