BLS, Board Certified in Internal Medicine or Family Medicine, CDS Certification, DEA Registration Must-Have:
Minimum one year experience with the frail or elderly population, Clean/Clear malpractice history, EMR TruChart experience highly preferred
- Serve as Outpatient PCP and skilled facility physician.
• Delivery of primary medical care to participants, including assessment, development of a plan of care in collaboration with the interdisciplinary team, provision of direct participant medical care, and evaluation of effectiveness of the plan.
• Make independent decisions regarding medical needs and health regimens; perform medical procedures as necessary.
• Promote and maintain a collaborative work environment with managers, peers, direct reports, and participants.
• Demonstrate and incorporate the organization's Mission, Vision, Values, and Guiding Behaviors in all actions and decisions.
• Reflect leadership skills, share strategy and vision, and lead organizational growth and change.
• Commit to participant and family independence and dignity through comprehensive care services.
• Embrace and lead in fostering a collaborative and consensus-driven work culture.
• Participate in an on-call rotation with approximately seven on-call days per month, including weekend telephone triage.
• Provide care and services that support seniors' independence in their homes for as long as possible.
